The question behind the search

Search for SEO not working and you will find eight-thousand-word technical checklists written for people doing SEO themselves. That is not you. You are paying someone, month eight has arrived, the invoices have cleared and the rankings have not moved, and the question in your head is simpler and harder: is this normal, is it me, or is it them? First, gather five pieces of evidence

No expertise required; everything below is visible in Google Search Console (ask for access if you do not have it, and worry if you are refused) and your agency’s own reports.

 

  • The shipped-work log. What was actually delivered each month: pages published, fixes made, citations built. A real programme has a list; a notional one has activity language.
  • Indexation. In Search Console, are the pages built for you actually indexed? Work Google has never seen cannot rank, and this failure hides for months.
  • The impressions trend. Clicks lag, impressions lead. Flat clicks with steadily rising impressions is a programme working; flat everything for six-plus months is not.
  • Long-tail movement. Before head terms move, question-style searches should be ranking. Their presence says the machine is turning; their absence at month eight says it is not.
  • Local metrics. For a clinic, Business Profile views, calls and direction requests usually move first of all. If even the local layer is flat, something fundamental is wrong.

The four explanations, and how each one looks

1. It is genuinely early for your market

Real in competitive specialties and central London, where nine to twelve months to meaningful positions is honest. But early comes with evidence: the work log is full, impressions are climbing, long-tail terms are appearing, local metrics are moving. If your agency says ‘these things take time’ and can also show you those four things, believe them and agree written milestones for the next quarter. If they say it and cannot, the phrase is doing a different job.

2. The scope never matched the goal

A few hundred pounds a month against established competitors in a saturated specialty was never going to move head terms, and a candid agency should have said so at the start. The tell: the work is real but thin, and progress exists only on minor terms. The fix is a re-scope conversation, up, down, or toward the local and long-tail battles the budget can actually win, not a change of provider.

3. Your clinic is the bottleneck

The uncomfortable one, and in healthcare the most common after the provider itself. Content queued for clinical sign-off for six weeks, website access never granted, decisions parked, engagement paused twice: each resets the clock everyone is complaining about. The tell is an agency work log full of items marked ‘awaiting client’. The fix costs nothing: a named reviewer with a standing weekly slot, access sorted once, and a fortnightly fifteen-minute decision call. 4. The provider is not doing the work

The signature: a thin or evasive work log, reporting that leads with rankings or traffic and never mentions enquiries, recycled generic content a clinician has clearly never reviewed, no Search Console access, and every question answered with ‘SEO takes time’. Any two of those together at month eight is your answer. In healthcare there is an aggravating factor: cheap tactics that quietly harm, thin AI-generated medical pages, junk links, which cost more to undo than they cost to buy.

The conversation to have before you decide

One meeting, requested in writing, with three asks: the complete work log since engagement start, Search Console access if you do not have it, and their own diagnosis of why movement is absent with a milestone plan for the next ninety days measured in the leading indicators above. A good agency will welcome this meeting; some will produce evidence that genuinely reframes the picture, and some will finally have the re-scope or bottleneck conversation that should have happened in month one. An agency that stalls, obfuscates or takes offence at being asked has answered the question by other means. Give the meeting a week to be scheduled and the plan a quarter to show leading-indicator movement, and put both deadlines in the follow-up email.

If it is time to switch, switch properly

Before giving notice, protect the ground you paid for: confirm in writing that you own the website, its content, the analytics and Search Console properties, the Business Profile and any ad accounts, and get admin access to each. Request the full work log and any content drafts as part of handover; check your notice period and serve it cleanly rather than mid-argument.
