Healthcare PPC Management Cost in the UK: Fees, Setup and Minimum Budgets

Healthcare PPC management cost has two separate parts: the fee you pay whoever runs your campaigns, and the ad spend you pay Google. UK sources put agency fees at roughly £1,000 to £1,100 a month on retainer, or 10 to 20 percent of spend, with the ad budget on top. Your minimum budget depends on local click costs and your booking targets.
This guide is for clinic owners who are pricing paid search for the first time or checking what they already pay. It focuses on how fees are structured and how to judge value. For how to build and run campaigns, see our guide to Google Ads for private clinics. For how we price paid media alongside our other services, see healthcare marketing pricing.
What is the difference between a management fee and ad spend?
Mixing these two up is the most common source of confusion in PPC quotes. They are paid to different people for different things.
- Ad spend is what Google charges for your campaigns. It goes to the ad platform, not the agency, and you control the limit.
- Management fee is what you pay the agency or freelancer for building, running and improving the account, and for reporting.
Expertmarket makes the same point in its UK cost guide: PPC has two costs, the agency fee and the platform fee. A quote that mentions only one of them is incomplete.
On Google Ads, you set a daily budget per campaign and can change it at any time. Google's help pages explain that daily spend can run up to twice your daily budget on a given day, but the month's charges are capped at your daily budget multiplied by 30.4. You also set bids, which are the most you are willing to pay for a click. The actual price is often lower.
How do agencies structure PPC management fees?
There are three common structures, plus a one-off charge for setup. Each shifts risk and incentives in a different direction.
Percentage of ad spend
The fee is a share of what you spend. Adzooma's review of UK agency pricing reports average PPC fees of 10 to 20 percent of ad spend. It is simple and scales with the account. The drawback is that the agency earns more when you spend more, whether or not results improve. Ask how the percentage behaves at higher budgets.
Flat monthly fee
A fixed fee for an agreed scope, regardless of spend. It is predictable and keeps incentives neutral. Check what the fee covers in hours or tasks, and whether it changes if you increase the budget or add campaigns.
Hybrid
A flat base fee with a percentage above a spend threshold, or a base fee plus a charge for extra services such as landing pages. In our experience hybrids work well when budgets are likely to change, provided the thresholds are written down clearly.
Setup or project fees
Some providers charge once to build the account, then move to ongoing management. Others fold setup into the first months. Neither is wrong, but you should know which you are agreeing to.
What do published UK PPC fee figures say?
We have not found a healthcare-specific UK source, so the figures below are general PPC prices. Expertmarket (updated March 2025) lists an average UK PPC agency retainer of about £1,100 a month, a project at about £1,200 and a day rate of about £800, with ad spend extra. Adzooma's UK review reports a PPC retainer average of about £1,040, a day rate of about £725 and the 10 to 20 percent of spend figure above.
Both sources describe averages without publishing a full method, so use them to sense-check a quote, not to set a budget. Healthcare accounts often need extra checking against advertising rules, which can affect scope. We do not publish Pulse Digital Health fees here because the right figure depends on how many services, locations and landing pages are involved.
What should PPC management include?
A monthly fee should buy active management, not just a login. Look for these items in writing.
- Keyword and search term review, including adding negative keywords to block irrelevant searches
- Advert copy testing within advertising rules
- Bid and budget adjustments
- Landing page recommendations, and changes if the fee covers them
- Conversion tracking checks for calls, forms and bookings
- A change log you can read
- Reporting that shows cost per enquiry and, where possible, per booked consultation
- A regular call with someone who knows your clinic
Ask what is not included. Landing page design, call tracking software, photography and the cost of your own staff answering enquiries are often separate. Our guide to checking your Google Ads agency lists questions to put to a current provider.
What setup costs should you expect?
Setup is where hidden costs appear. Before a campaign runs, someone has to do the work below, and your quote should say who and at what price.
- Account and campaign structure, built around the services you want to grow.
- Conversion tracking for calls, forms and booking steps, tested end to end.
- Landing pages that match each service, so visitors arrive on the right page.
- A review of the ads and pages against Google's healthcare policy and UK advertising rules.
- Access and ownership: the Google Ads account should be in your name.
On the policy point, Google restricts some healthcare content in the UK, including prescription drug services and some categories such as addiction services and abortion-related queries, which need certification. Check Google's current Healthcare and medicines policy for your service, and see our guide to Google Ads compliance for UK clinics. This is general guidance, not legal advice.
What is a realistic minimum budget for a private clinic?
No honest single number exists. Adzooma reports that agency minimum spends range widely, from £400 a month to £10,000, which shows how much depends on the provider and market. A better approach is to work out your own minimum from four inputs.
- Target bookings per month for the service you want to grow.
- Enquiry-to-booking rate, from your own records.
- Click-to-enquiry rate of your landing page, measured once tracking is live.
- Cost per click for your terms and area, from Google's Keyword Planner.
Multiply backwards: bookings, then enquiries, then clicks, then clicks times cost per click. That is the spend needed to reach your target, before the management fee. If the result is far above what you can afford, narrow the service, area or goal rather than spreading a small budget across many campaigns.
Very small budgets can make learning slow, because there are few clicks to judge. Our articles on starting ads on a small budget and why cutting a budget to almost nothing backfires cover the trade-offs.
How does tracking change what you are paying for?
Without reliable tracking, you cannot tell whether the fee or the spend is working. Counting button clicks can overstate results, so ask how a call, a form and a booking are each recorded. Our guide to click-to-book overcounting shows what to check.
The stronger set-up passes real bookings back to Google, as explained in our piece on offline conversions. Check whether this is included in the fee or sold as an extra, and whether handling patient data has been reviewed for privacy.
How do you judge whether PPC management is good value?
Value is not the lowest fee. It is what you learn and gain for the total of fee plus spend. Use a short monthly check.
- Cost per booked consultation, not only cost per click or per enquiry.
- Search terms report: are patients searching for your services, or are you paying for irrelevant clicks?
- A written change log that explains what was changed and why.
- Clear notes on what was tested and what the next test is.
- Landing page conversion, with a plan if it is weak.
- Whether the numbers in the agency report match your own booking records.

Healthcare PPC management cost: how to decide
Treat the fee and the ad spend as one investment, and set a total you are comfortable testing for a fixed period. Choose a fee structure that matches your plans: a flat fee for a stable budget, a percentage or hybrid if spend will move. Insist on account ownership, readable reporting and tracking that reaches bookings.
Before you sign, make sure the quote states the fee, the minimum term, the setup charge and who owns the account. To see how paid search is delivered alongside the rest of your marketing, visit our Google Ads for healthcare page.
Frequently asked questions
How much does healthcare PPC management cost in the UK?
No healthcare-specific UK survey exists that we found. General UK sources put agency PPC retainers at about £1,000 to £1,100 a month, or 10 to 20 percent of ad spend, with the ad budget paid separately. Healthcare quotes depend on services, locations and compliance checking.
Is the management fee included in my Google Ads budget?
No. Your ad spend goes to Google and the management fee goes to your agency or freelancer. Ask for both figures in the quote, plus any setup charge, so you can see the total monthly cost before agreeing.
Is a percentage of ad spend or a flat fee better?
Neither is always better. A percentage scales with the account but pays the agency more when you spend more. A flat fee is predictable. A hybrid suits changing budgets. Choose the one where scope, thresholds and incentives are clearly written down.
What is the minimum Google Ads budget for a private clinic?
Google Ads has no single required minimum for clinics. Agencies set their own minimums, which Adzooma reports range from £400 to £10,000 a month. Work backwards from target bookings, conversion rates and cost per click to find your realistic figure.
Are there setup fees for Google Ads management?
Often, though some agencies include setup in early monthly fees. Setup covers account structure, tracking, landing pages and policy checks. Ask what is charged, what you receive and who owns the account, so there are no surprises later.
How do I know if my PPC agency is worth the fee?
Look at cost per booked consultation, the search terms report, a written change log and whether agency numbers match your own booking records. A good provider explains what changed and why, and flags problems early.
Do healthcare ads cost more because of Google's policies?
Policies do not set your click prices, which come from the auction. They can affect scope and time, because some healthcare content is restricted or needs certification in the UK. Check Google's Healthcare and medicines policy for your service before building campaigns.
How long should I commit to a PPC contract?
Long enough for a fair test, but not so long that you are locked in without review points. Agree milestones, a notice period and what happens to the account and data if you leave. Avoid contracts where you do not own the account.
